For valuable consideration received, I (the applicant) grant YES House Foundation, their assigns, licensees, and legal representatives the irrevocable right to use his or her picture, portrait or photograph in all forms, media, and promotional materials without
restriction as to changes or alterations for advertising, trade, promotion exhibition or any other lawful purpose. I (the applicant) waive any right to inspect or approve of the photograph or electronic matter that may be used in conjunction with them now or in the future, whether that is known to e the applicant or not, and guest waives any right to royalties or other compensation arising from or related to the use of the photographs
I understand and agree that YES House Foundation website, internal files, and records, including names and addresses of volunteers, donors, membership lists and similar ate are the sole property of YES House Foundations and that unauthorized disclosure of such information, its appropriation, changes or alterations to it may constitute a criminal offense
I hereby waive and release, indemnify, hold harmless and forever discharge YES House Foundation and its agents, officers, directors, affiliates, successors and assigns of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, of every kind of nature, whether known or unknown, in law or equity, that I ever had or may have arising from or in any way related to my participation in any o the events or activities conducted by, or for the benefit of, YES House Foundation
